    In October 2001, Michelle and Simon McWilliam, foster parents in Fishersgate, West Sussex, were sentenced to eight years in prison for child cruelty after the death of four-year-old John Smith. John had suffered months of brutal treatment, culminating in a fatal brain haemorrhage. Social workers failed to intervene effectively despite being aware of numerous injuries, including 54 visible wounds and three adult bite marks, with the child's hair also partially missing.

    John died on Christmas Eve 1999, with paramedics discovering him unconscious, clutching his favorite toy dinosaur. Simon McWilliam described the hospital admission as 'inconvenient' because it coincided with the end of his university course. Both he and Michelle, who was an accountancy student at the time, later fabricated stories about the injuries.

    Judge Anthony Scott-Gall condemned the couple, stating, 'You systematically set about abusing that little boy, who wanted no more out of life than to be loved and to give love. You made a mockery of those wishes.' He emphasized that the couple had lied to teachers and social workers, calling them 'cruel and heartless.'

    An independent report highlighted that social workers visited the McWilliams' home 20 times during the six months John was in their care. On at least seven occasions, injuries were suspicious enough to warrant medical investigation, yet Brighton and Hove Council staff accepted false explanations provided by McWilliam and his third wife, 35-year-old Michelle. The couple, who had applied to adopt John, insisted he had caused his injuries himself, claiming he frequently threw himself downstairs and into furniture, labeling him a 'problem child.'

    Social worker David Pamely believed these accounts despite John having no prior history of self-harm. Before his placement with the McWilliams, John had spent 17 months with foster parents Rod and Pauline Mears, who described him as cheerful and well-behaved. Born in May 1995, John was removed from his mother Marion Smith, who was unable to cope and had attempted suicide. His biological father, previously charged with child abuse, was barred from the family home by social services.

    In summer 1999, after a thorough but insufficient assessment, the social services decided John would be better matched with the McWilliams, who sought to adopt following failed fertility treatments. A report by social services expert Alyson Leslie criticized the process, noting that few checks were made on the personal information provided by the couple, and that contacts with McWilliam’s ex-wives revealed concerns about his cruelty and violence.

    Throughout their assessment, the couple’s stories about John’s injuries remained unchallenged by officials. The first report of injuries emerged a month after John moved in, with Michelle McWilliam claiming he had bumped into a table and slid down stairs. Later, in October, social workers saw a carpet burn on his face, and in December, he was found with a black eye and bruised face. Despite these signs, concerns were downplayed.

    In December, John’s injuries escalated. On December 23, Michelle McWilliam called for an ambulance. Paramedics found John unconscious, clutching his toy, and discovered multiple injuries. His foster parents later criticized social workers for ignoring their pleas for help.

    Following the trial, John’s natural parents expressed their shock and sorrow, stating they 'cannot begin to understand why their son was placed with people such as the McWilliams.' John's first foster mother affectionately called him her 'Little Star' and 'a rainbow who touched everybody.' Meanwhile, the tragedy highlighted ongoing failures in social services, paralleling other tragic cases such as eight-year-old Anna Climbie and six-year-old Lauren Wright, both victims of neglect and abuse despite previous warnings to authorities.
